2nd - 8th Grade
I use this daily behavior slip with my middle school students who have a hard time with intrinsic motivation. They must give the slip to their teacher at the start of the period and turn it in at the end of the day for a small prize. So far, I have seen great results with the students who "buy in" to the system.
Space for 6 periods
Categories include:
On time
Prepared
Following along and on task
Respectful
Teacher's signature
Total
Scale: 2 - excellent 1 - needs improvement 0 - did not meet
I usually do not expect my students who require this slip to receive a perfect score to earn a prize. That is why there is a percentage line. If they get 80% (also a good math practice tool), then they receive a small prize or sticker.
