5th - 12th Grade
As a counselor, you have numerous superpowers. Some days, your superpower is accurate and relentless advocacy for your most vulnerable students. Students struggling with suicidal and homicidal thoughts deserve access to supportive, appropriate, quality mental health care. This form does double duty: it provides documentation about why you recommend an emergency eval, and it provides documentation the family can take to the hospital. Sometimes quality documentation from a counselor the child trust provides insight an ER interview won't, which means sometimes documentation like this is the difference between your student getting the help they need and your student being sent home without additional resources. This will be a forever freebie.
