ALL ABOUT ME Student Information Form/Survey for Back to School (Editable)


Are you tired of plain student questionnaires on the first day of school? Use this two-pager back-to-school student information form to learn more about your secondary students. A fully editable version is also included, so you can adapt this resource to better suit your needs. 
Use this resource to learn more about:
  • students' qualities, pet peeves, interests
  • the types of novels, music genre, films, TV shows, artists they like
  • learners' strengths and difficulties (ESL skills/competencies: speaking, reading, writing, grammar) - You can change these using the editable version.
  • students' learning style
  • academic accommodations they had the previous year (e.g. extra time, computer for writing)
This student form also contains a written assignment section that you can use to evaluate your learners' writing skills.
This resource includes Canadian spelling.
